Reference thresholds & caveats
- Acetaminophen (Paracetamol): Evaluate ≥150 mg/kg. High risk ≥200 mg/kg. Always obtain 4–24 h serum level for nomogram.
- Elemental Iron: Evaluate ≥20 mg/kg; significant ≥40 mg/kg; severe ≥60–80 mg/kg.
- Salicylate (Aspirin): Mild ≥150 mg/kg; moderate ≥300 mg/kg; severe ≥500 mg/kg.
- Ibuprofen: Evaluate ≥200 mg/kg; severe risk ≥400 mg/kg.
- Diphenhydramine: Evaluate ≥7.5 mg/kg; severe ≥20 mg/kg.
- Clonidine: Evaluate ≥0.01 mg/kg (10 µg/kg); severe ≥0.02 mg/kg.
- Sulfonylureas, Opioids, TCA, CCB, Highly‑toxic β‑blockers (e.g., propranolol): Any ingestion → medical evaluation; extended‑release ↑ risk.
- Camphor: Evaluate ≥30 mg/kg; severe ≥50 mg/kg.
- Thresholds are screening guides; clinical context, formulation (IR vs ER), co‑ingestions, and comorbidities matter.
