Management Guidance

Supportive Care
  • Monitor vital signs continuously
  • Establish IV access and maintain hydration
  • Monitor electrolytes, glucose, ABG, LFTs
  • GI decontamination if recent ingestion (consider whole bowel irrigation for sustained-release formulations)
Deferoxamine Chelation Therapy

Indications:

  • Severe toxicity (>60 mg/kg elemental iron)
  • Metabolic acidosis
  • Shock or altered mental status
  • Very high serum iron levels (>500 mcg/dL)
  • Significant GI symptoms with systemic signs

Dosing:

  • Initial: 15 mg/kg/hour IV infusion (max 6-8 g/day)
  • Duration: Continue until clinical improvement and serum iron normalizes
  • Monitoring: Watch for hypotension, ARDS, and vin rosé urine (indicates ferrioxamine excretion)
Disposition
  • <20 mg/kg: Observation for 4-6 hours; discharge if asymptomatic
  • 20-60 mg/kg: Admit for observation and monitoring
  • >60 mg/kg: ICU admission for aggressive supportive care and chelation therapy
